Position Summary
The Patient Navigator works collaboratively with all team members to navigate patients through the healthcare system, which ensures quality care is delivered effectively based on individual patient needs and assets.  Acts as the first point of contact for the clinic and works with National Jewish Health providers, community clinic providers, and patients to seamlessly coordinate care. Works to promote efficient, high-quality clinical practice by providing guidance through collaboration with the team to manage patient flow and communication.

Essential Duties

  1. Performs clinical and administrative skills, including scheduling of oncology patients (new and follow-up). Retrieves all pertinent data for oncology and hematology patients prior to the visit with a provider, including, but not limited to; Outside pathology reports, ensuring slides are sent to appropriate departments for reading and diagnosis, outside images and reports, and pertinent outside notes or other documents from referring providers.
  2. Assists oncology patients with supportive cancer services through local or national assistive programs, examples including insurance, transportation, housing, nutrition, and medication.
  3. Educates patients and caregivers on the interdisciplinary nature of cancer treatment and the roles of team members.
  4. Identify barriers to care such as transportation, childcare, eldercare, language, etc., and facilitate referrals as appropriate to mitigate barriers.  Support transition of patients across screening, diagnosis, active treatment, and survivorship working with all members of the team. 
  5. Coordinate the plan of care with the interdisciplinary team, promoting timely follow-up on treatment and supportive care recommendations.  Monitor and facilitate interventions to address symptoms and side effects.  Help patients adhere to treatment plans.  Help patient understand ongoing testing results. 
  6. Provides support for oncology patients by collaborating and communicating with health care team members, oncologists, chemo infusion centers, and patients. Navigates patients through the entire cancer experience interacting with other colleagues, and organizations to promote the best interest of patients.  
  7. Prepares cases for the Oncology Tumor Board and ensures all reports are ready for Oncology Tumor Board on a weekly basis. Distribute materials to Tumor Board members.  Facilitates communication with the patient regarding the smooth transition of care between services.  
  8. Represents department by participating in and may function as a reviewer in committees, boards, skills review, skills lab, candidate interviews, and special projects as assigned.
  9. Assists the Oncology division in research, which may include maintaining up-to-date data for the Lung Nodule registry and/or assisting in data collection.

Minimum Qualifications

  1. Education: High school diploma/GED and successful graduation from an accredited Medical Assistant program required.
  2. Work Experience: Minimum of 2 years experience as a Medical Assistant required..
  3. Special Training, Certification or Licensure: CPR/BLS required. Limited Scope is preferred for specific clinics. Navigation certification preferred.
     

Salary Range: $21.00 - $26.50
