Position Summary

 

Responsible for ensuring that referrals and consultations from individuals waiting to become new patients, follow up for past patients, or physicians interested in referring a patient are efficiently and appropriately handled. The ID Intake Coordinator is essential to collecting relevant and timely information so that the Infectious Disease physician can make informed, timely and appropriate decisions related to patient care. The ID Intake Coordinator will be accountable to maintain all patient records required for Physician/NP to view to ensure they are making the correct decision in developing a care plan for the infectious disease patient.

 

 

Essential Duties

 

1. Facilitates self and physician referrals to Infectious Disease at National Jewish Health by quickly and efficiently obtaining in depth patient records and health histories from patients, outside physicians, and laboratories throughout the country. Reviews records to ensure complete patient history and appropriate case for the Infectious Disease Department. 

 

2. Responsible for maintaining and updating a database with patient health information including their demographics, referral physician information, relevant information on past patients as well as potential new patients. 

 

3. Responsible for presenting and discussing detailed patient history and intake information at weekly multidisciplinary meetings with department clinical practitioner to decide how soon to see the patient and the most appropriate level of care for the patient. 

 

4. Coordinates telephone and written communication with patients, referring providers and other members of the health team to keep them apprised of the status of the patient's admission.

 

5. Communicates to patients and physicians the expectations of the Infectious Disease Program and utilize a follow-up system to assure coordination of visit and consultation requests are accomplished.

 

 

Other Duties

 

1. May assist with coordination of the collection process for laboratory samples by mailing appropriate collection kits to patients and referring physicians. 

 

2. Assist with chart preparation for upcoming appointments.

 

3. Provide back up support to department team during staff absence and may perform clinical skills as guided by competency validation.

 

4. Other duties as assigned.

Minimum Qualifications

 

Education

 

High school diploma or equivalent required.  Successful graduation from an accredited Medical Assistant Program preferred.

 

Work Experience

 

A minimum of two years of recent and related experience within a healthcare environment 

 

Special Training, Certification or Licensure

 

BLS/CPR required.  

 

Salary Range: $17.00 -$22.00
