Summary

Position Summary    

The Nurse Coordinator is essential to the care of patients with the most complex care needs that are at-risk of experiencing the greatest illness/disease burden and high health care service utilization.  The Nurse Coordinator is responsible for meeting with these patients/families to mutually develop a patient’s interdisciplinary action plan that operationalizes the plan of care through identifying the necessary steps to be completed. The Nurse Coordinator follows-up with the patient to monitor adherence, progress and barriers to the plan.  Works with the patient and clinic team to revise the plan as necessary and is instrumental in communication and coordinating care among the clinic’s interdisciplinary team.


Essential Duties    

  1. Conducts comprehensive, multidisciplinary assessments by evaluating all relevant information to determine individual needs and barriers in health care, social supports and access to behavioral health services.
  2. Develops individualized treatment/action care plans based on comprehensive, multidisciplinary assessments.
  3. Regularly review the patient’s progress toward goals through face to face consultations, telephone, text, email, and other appropriate forms of communication.
  4. Coordinates with various service providers to ensure the fulfillment of patient’s needs.
  5. Participates in pre-clinic huddles to prepare for clinics and facilitate communication and coordination between health care team members.
  6. Maintains up-to-date records documenting care, progress toward action plan goal.
  7. Manages patient assignments, set priorities, adjust effectively to changes in priorities and circumstances, organize clinic flow, and utilize available resources.
  8. Provides verbal and written communication to the clinic’s multidisciplinary team and NJH consults including recommendations to improve patient’s participation in clinic and achievement of individual patient goals such as treatment course, psychosocial assessment, and disposition planning.
  9. Works collaboratively with interdisciplinary health care team members both internally to National Jewish Health and externally to improve patient care through effective utilization and monitoring of health care resources. 
  10. Participates in the development, data collection, and analysis of team-based quality improvement projects.
  11. Supports research and quality improvement projects that will contribute to improving patient-care, staff development and the mission of the institution.
  12. Actively seeks out and shares current research &/or clinical practice guidelines to direct current direct patient care practice.

Minimum Qualifications

  1. Education: Graduation from an Accredited School of Nursing required, BSN preferred.
  2. Work Experience: A minimum of 2 years of recent and related Registered Nurse (RN) experience required.  Different patient care areas may require specific skill set experience.
  3. Special Training, Certification or Licensure: Current Colorado Registered Nurse (RN) License and current CPR/BLS certification required.  ACLS or PALS if applicable for specific unit required.  Certification in a relevant specialty preferred.

 

Salary Range: $35.00 - $40.00
